How to Add Notes
1. From the Dashboard, select the individual's name.
2. Select "More" on the right side and click "Notes".
3. A form will appear on the right where you can add your notes and select a note type.
Some of the most common Note types are below:
• General - A note for general use.
• Milestone Event - These notes will display a line on graphs/charts.
• Psych Consult - A note for a consult with another member of the care team.
• Treatment Plan - You can modify an individual's treatment plan directly from notes, which will then transfer to Monthly Reviews. A treatment plan is intended to be used as one ongoing document. Therefore, when you make a "new" Treatment Plan note, the contents of the last note will automatically appear. You also have the ability to modify this note type later, and print it as a PDF.
• Flag - If you add a flag to an individual, you can add notes to those flags. In that case, we recommend selecting "Flag" as the note type.
4. After entering a note, you can select "Save" if the note is not complete, or "Save & Sign" if the note is complete and ready to be published.
How to View Notes
- On the patient's profile page, click "Actions" located to the right of the individual's name and select "Notes".
- A form will appear on the right with an "ALL NOTES" section where you can view past notes.
From here, you'll be able to copy notes, print notes, and, depending on the note type and your role, delete notes.
