The Opioid Risk Tool (ORT-OUD) and the Prescription Drug Use Questionnaire (PDUQp) are an available option.
The Opioid Risk Tool (ORT-OUD, used similarly to SOAPP-R) is designed for individuals who have not yet started opioid treatment to assess risk for opioid prescription. Generally speaking, this will not change over time because it is looking at medical history.
The Prescription Drug Use Questionnaire (PDUQp, used similarly to COMM) can be used as regular assessments for opioid patients as check-ins.
These questionnaires can be administered like any other questionnaire, either in person or assigned directly to the individual.
How to Administer the Questionnaire In Person
1. Within the Client Dashboard, select "Actions" > "Questionnaire"

2. This will take you directly to the page for you to choose the assessment you would like to do:

3. Once you choose the questionnaire, you will then need to choose how you would like to administer the questionnaire.

4. If you choose to take on behalf of the individual, you will be taken to begin the questionnaire:

If you would like to assign the questionnaire to an individual, simply assign the ORT-OUD or PDUQp directly to the client:

What Happens Next?
After completing the questionnaire, you'll be able to find it in the "Activities" table at the bottom of the Client Dashboard. Additionally, you can click the questionnaire to view the detailed answers, and download a PDF if desired for record-keeping.
NOTE: The results of the opioid questionnaires will also appear on the Monthly Review, just like any other questionnaire. However, the completion of these questionnaires will NOT show a Monthly Review notification in the Care Team Dashboard.
Hot Tip: The assignment of this questionnaire (as well as any other activity within NeuroFlow) is available for automatic assignment via Triggers.
