To add a new individual, simply click on the "Clients" or "Patients" drop-down menu on the top left of the Care Team Dashboard (it may also say Members, Individuals, or another version thereof, we will use Member for the remainder of this example). Select "Add Member" and a pop-out window will slide out from the right side of the screen.
Required: First and Last Name, Date of Birth, Gender, Internal Care Manager (individual's provider), Current Program, and confirmation of consent(depending on your organizations specific settings).
In order to make registration easiest for the individual, we recommend you enter the individual's phone number & email. If the patient has not expressed consent to receive text messages, you can express this by switching the consent toggle off. If they have expressed consent, the "Send Invite" button will send both a text message and email with a link directly to that person. You can also choose at account creation if the invitation is sent via text, email, or both.
NOTE: Depending on your settings, the Add Member page may have fewer fields.
IMPORTANT: Please make sure you enter the correct date of birth and email. The individual will need to enter these in order to complete registration.
