You can pull several reports from NeuroFlow, including an Outcomes Report!
The Outcomes Report provides insight into patient outcomes for PHQ-9, GAD-7, AUDIT-C, DAST-1/10, Oswestry (Back & Neck), and SQS data. The report can inform outcomes, improvement, and provider performance for certain organizations. We hope this report gives you a better sense of your patients' journey and progress.
The Outcomes Report also includes information about Urgent / At-Risk Alerts and the ability to choose which questionnaires to include, as well as which tags have been applied to patients. To learn more about tags, see this article, Custom Tags in The Care Team Dashboard.
As you begin to use the Outcomes Report, you can use this article as a reference for column definitions within the report. (Note: EngageBH does not have access to the Outcomes Report)
How to Run the Outcomes Report:
1. To generate the report, log into the NeuroFlow platform and click on 'Reporting' at the top of the care team dashboard, then select 'Outcomes Report (CSV)'.
2. A menu will then appear giving you the option to create your report with specific patient-related and time-related parameters.
Other parameters include patients under specific providers and which questionnaire data to include.
3. After you click 'Create Report,' s CSV file will be generated. Click “Download CSV” to open the downloaded file from the notification. Don't worry if you miss it! You'll also receive an email with this information.
Note: Be sure you are logged in to NeuroFlow when you click the "Download" link in your email.
Within the file, you will see several columns with the following information:
- DOB - Birthday in (format: MM/DD/YY)
- Active Flag - Active flag on patient profile
- Patient ID - Patient MRN (if applicable)
- Prescriber - prescriber's first and last name

Care Manager - bh specialist first and last name

Current Program - Current patient program (BHI, CoCM)

Current CoCM Status - Current Status (Active, Pending Consent, Discharged)

CoCM Discharge Reason - Discharged reason (Graduated, Maintenance Planning)
- Urgent Alert - 'Yes' or 'No' if the individual has triggered an Urgent Alert
- Latest Urgent Alert Date - The latest date that the individual triggered an Urgent Alert

At Risk Alert - 'Yes' or 'No' if the individual has triggered an At Risk Alert
- Latest At-Risk Alert Date - The latest date that the individual triggered an At-Risk Alert

Remission (PHQ-9 <5) - is TRUE if any PHQ-9 for given patient after a previous PHQ-9 >= 10 is less than 5, else FALSE

Time to 50% Reduction PHQ-9 - if the patient had a PHQ-9 >=10 AND had a 50% reduction in their score; gives the amount of time for the first 50% reduction in PHQ-9 scores (format: ‘1 yrs 7 mo 6 days’)

Ex. if first PHQ-9 taken is 10 on 5/1/19 and second PHQ-9 taken is 5 on 6/1/19, then Time to 50% Reduction PHQ-9 = 1 mo 0 days

If a patient does not meet the criteria of PHQ-9 >=10 and 50% reduction, this is left blank

GAD-7 First Score - first recorded GAD-7 score in NeuroFlow for a given patient

GAD-7 First Date - date of first recorded GAD-7 score, in MM/DD/YYYY format

GAD-7 Last Score - most recent recorded GAD-7 score in NeuroFlow for a given patient
- GAD-7 Last Date - date of the last recorded GAD-7 score (format: MM/DD/YYYY)

Time Between First & Last GAD-7 - the amount of time between GAD-7 First Score and GAD-7 Last Score (format: ‘1 yrs 7 mo 6 days’)

Remission (GAD-7 <5) - is TRUE if any GAD-7 for given patient after a previous GAD-7 >=11 is less than 5, else FALSE

Date Oswestry (Back/Neck) Remission Achieved - is TRUE if Oswestry (Back/Neck) remission has been achieved

Date Oswestry (Back/Neck) 50% Reduction Achieved - date 50% reduction in Oswestry (Back/Neck) was achieved

Activity Count Between First and Latest Oswestry (Back/Neck) - number of activities completed between first and last score for PHQ-9 and GAD-7

Ex. - “75” = 75 activities completed

Daily Activity Rate Between First and Latest Oswestry (Back/Neck) - the ratio of the number of activities to days between the first and last score for PHQ-9 and GAD-7

Ex. - “3.00” = 75 activities over 25 days = 3.00 ratio

- Tags - Each column will contain category names. Each cell will contain tags under the category separated by commas
